To qualify for assistance from this fund, applicants must meet all of the following criteria.

Patient has been diagnosed with Leukemia within the last year.

Patient lives and owns their home in the state of Louisiana.

Completed Cinnamon Helps Patient Authorization form.* Please submit authorization form when you submit your request.

**This is a Health Equity fund, there are geographic restrictions that apply.**
